Three different Indian government agencies tracking the same period produced three different death counts, sometimes by a factor of two or three. For emergency planners and occupational health teams, that gap is not a statistical footnote. It is the difference between preparing for a manageable seasonal risk and preparing for a mass casualty event that is already underway.
A reporting system that produces conflicting numbers
Data obtained through India's Right to Information Act shows 3,812 heatstroke deaths recorded between 2015 and 2020 by the National Centre for Disease Control. Over the same period, the National Crime Records Bureau reported 8,171 deaths from heat and sunstroke, while the India Meteorological Department recorded 3,436 heatwave deaths. Three agencies, three figures, no shared registry.
The pattern held through 2025. The NCDC reported 7,192 suspected heatstroke cases between March 1 and June 24, 2025, but confirmed only 14 deaths nationally. Independent monitoring by the non-profit HeatWatch, which tracks media-reported fatalities, identified at least 84 heatstroke deaths across the same broader period. Neither figure captures the full picture: clinicians and public health researchers point out that heat rarely appears as the primary cause of death on a certificate, even when it triggered the cardiac arrest, kidney failure, or stroke that killed the patient.
What the excess-mortality models suggest
When researchers move past confirmed heatstroke codes and instead measure excess deaths during heat events, the numbers change by an order of magnitude. One national study estimated that a single severe heatwave day can produce roughly 3,400 excess deaths across the country. Independent academic reviews estimate that India has accounted for close to a quarter of all heat-wave-related excess deaths recorded globally since 1990. Only about 8% of Indian households have access to air conditioning, which leaves a large share of the population physiologically exposed during peak heat events with no mechanical relief available.
The exposure is not confined to the regions typically associated with extreme heat. During the 2025 heatwave season, fatalities were reported in Assam and in Himalayan states not historically considered high-risk, and temperatures in parts of Rajasthan reached 48°C.
The occupational and outdoor-labor dimension
A large share of India's workforce, agricultural laborers, construction workers, delivery and logistics staff, works outdoors during the hottest hours of the day, often without formal heat-safety protocols or scheduled cooling breaks. This is the population most likely to develop exertional heat stroke rather than the classic, heatwave-driven form seen in elderly or sedentary victims. Exertional heat stroke has a narrower treatment window: core temperature needs to come down fast, and the type of care available in the first 30 minutes after collapse determines the outcome far more than anything that happens after hospital arrival.
Closing the treatment gap in the field
The clinical evidence on this point is consistent across countries with very different climates and health systems. The Korey Stringer Institute's dataset of over 401 exertional heat stroke cases shows a 100% survival rate when core temperature is brought below 40°C within 30 minutes of collapse, using cold water immersion. Cold water immersion cools the body at roughly 0.35°C per minute, about ten times faster than wet towels or ice packs, which is what makes the treatment window achievable in a field setting rather than only in a hospital.
