Japan does not top the global list for total heat deaths. It tops the list for the rate at which heat kills, by a wide margin, and that distinction matters for how emergency and public health systems should read the data.
The comparative numbers
A multi-country study covering 34 nations between 2000 and 2022, using standardized ICD-10 heatstroke codes, found Japan's heatstroke mortality rate to be the highest in the dataset at 5.81 deaths per million population, ahead of Cyprus at 2.51 and China at 2.42. Most countries in Europe, South America, and Southeast Asia reported rates below one death per million. The same research found that heatstroke deaths account for nearly a quarter of all heat-attributable mortality in Japan, far above the low single-digit percentages seen in most other countries studied.
The absolute figures back up the pattern. Japan's Ministry of Health, Labour and Welfare recorded over 1,500 heatstroke-related deaths in recent years, with 2024 setting a record above 2,000. The summer of 2025 pushed the strain on emergency services further: more than 100,000 people were transported to hospital by ambulance for heatstroke between May and September, the highest figure since national record-keeping began, a nearly 3% increase on the previous year's total.
Why Japan's numbers stand out
Two structural factors explain much of the gap. Japan has one of the world's oldest populations, and heatstroke mortality skews heavily toward people over 65, particularly those living alone or without air conditioning running consistently. Second, summer temperatures have been rising measurably: the national average summer temperature in 2024 was the highest recorded since 1898, with the record broken again during the summer of 2025.
The response has been correspondingly structural. In 2023 the Japanese government adopted a formal Action Plan for heatstroke countermeasures with an explicit mid-term target: cut heatstroke mortality in half from current levels by 2030. In 2024 it introduced a tiered "Special Heatstroke Alert" system, distinct from ordinary heat advisories, reserved for disaster-level heat events.
A model other countries are starting to reference
Japan's experience is increasingly cited internationally, including by public health researchers in India, precisely because it demonstrates that heat mortality is not simply a function of how hot a country gets. It is a function of demographics, housing and cooling infrastructure, and how fast the emergency response system can act once heat illness starts. Countries with younger populations and lower baseline heat exposure should not assume Japan's high rate is irrelevant to them; it is closer to a preview of what happens when an aging population meets rising summer temperatures without a matching increase in field-level response capacity.
Where field response still lags behind policy
Alert systems and mortality targets address prevention and public awareness, but they do not by themselves change the outcome once someone has already collapsed from heat illness. That outcome is decided in the first 30 minutes, and it is decided by how fast core body temperature comes down, not by which hospital the patient eventually reaches.
