Every fire service in the world faces the same physiological problem: firefighters generate extreme internal heat load in insulated protective gear, and that heat has to come out safely before it becomes exertional heat stroke. How each country writes that requirement down, and whether it's binding, varies enormously. For departments benchmarking their own rehab procedures, or evaluating equipment against a named standard, the comparison is worth laying out clearly.
United States: NFPA 1584
NFPA 1584, Standard on the Rehabilitation Process for Members During Emergency Operations and Training Exercises, is the most explicit of the four frameworks compared here. First issued in 2003 as a recommended practice, it was upgraded to full standard status in 2008 and most recently revised in 2022, with new chapters added on preliminary exposure reduction and post-incident recovery.
The standard requires rehabilitation after specific exposure thresholds (two 30 or 45-minute SCBA cylinders, or one 60-minute cylinder), and sets out a two-tier cooling requirement:
- Passive cooling first: shade or air conditioning, PPE removal, cool fluids, rest
- Active cooling when passive cooling is insufficient or a member shows signs of heat-related illness, explicitly naming forearm immersion, misting fans, and similar methods
In hot, humid conditions, NFPA 1584 calls for a minimum of 10 minutes of active cooling, with 20 minutes preferable. Release from rehab has defined vital sign benchmarks: core temperature below 100.6°F (38.1°C), heart rate below 100 bpm, blood pressure below 160/100, and SpO2 above 94%.
What makes NFPA 1584 unusual internationally is that it names a specific cooling method, forearm immersion, inside a consensus standard. Whole-body cold water immersion, the method with the fastest documented cooling rate, meets and exceeds this requirement.
United Kingdom: NFCC National Operational Guidance
The UK has no single numbered standard equivalent to NFPA 1584. Instead, the National Fire Chiefs Council (NFCC) publishes National Operational Guidance, including a dedicated module on preventing and managing heat illness in personnel, as part of a broader library of operational good practice that individual fire and rescue services adapt to local policy.
This is guidance rather than a codified, independently enforceable standard: each UK fire and rescue service is expected to translate NFCC guidance into its own local procedures, which means implementation detail, including what cooling equipment is actually on hand at incidents, varies by service in a way it does not under a single national standard.
Australia and New Zealand: AFAC guidance
Across Australia and New Zealand, the AFAC (formerly the Australasian Fire and Emergency Service Authorities Council) plays a coordinating role comparable to the NFCC's in the UK. Its Managing Heat Stress in Emergency Response guideline, developed by AFAC's Workplace Health and Safety Technical Group, gives fire and emergency service organisations a framework to build heat stress procedures on, while leaving implementation to individual agencies and to the differing work health and safety legislation across Australian states and New Zealand.
Notably, some of the most cited research on firefighter cooling via water immersion, including Brearley and Walker's 2015 review of fireground cooling modalities, originates from Australian fire and trauma research institutions, giving the region a strong evidence base even without a single binding standard.
France: no single national equivalent
France has no direct counterpart to NFPA 1584. Firefighter (sapeur-pompier) rehabilitation and cooling practice is instead shaped by a combination of national clinical guidance from emergency medicine bodies, operational doctrine issued by the DGSCGC (Direction générale de la sécurité civile et de la gestion des crises), and procedures set at the SDIS (departmental fire service) level. In practice, this means two SDIS departments can run meaningfully different cooling protocols for the same type of incident.
Décret n°2025-482, in force since July 2025, moves France toward the US and Australian model by making certain heat-illness prevention measures for emergency responders mandatory rather than advisory, but it does not consolidate rehab procedure into a single named standard the way NFPA 1584 does.
What this means for equipment decisions
Regardless of which framework applies, the underlying physiological requirement is identical: get core temperature down fast, with full access to the patient for monitoring. Departments operating under NFPA 1584 have the clearest documented hook, since whole-body immersion satisfies the active cooling requirement more completely than the fan- or towel-based methods often used as a default. Departments in the UK, Australia and New Zealand, or France, working from guidance rather than a single binding standard, have more latitude, but the same clinical case applies regardless of which document governs the decision.
