NFPA 1580, Standard for Emergency Responder Occupational Health and Wellness, now carries the rehabilitation requirement that used to stand alone as NFPA 1584. For a department building or updating its rehab sector, the standard sets specific thresholds, not just a general instruction to rest crews between assignments. This guide walks through what NFPA 1580 requires, how to lay out a rehab sector that meets it, and where an immersion-capable cooling tub fits in the cycle.

What triggers rehab under NFPA 1580

The standard sets exposure thresholds that trigger mandatory rehabilitation: two 30 or 45-minute SCBA cylinders, or a single 60-minute cylinder. A member who crosses that threshold enters rehab before returning to assignment, regardless of how they report feeling. Self-assessment is not a substitute for the threshold, since firefighters under exertion routinely underestimate their own heat load.

The two-tier cooling requirement

Passive cooling comes first for most members: shade or air conditioning, PPE removal, cool fluids, rest. This is sufficient when a member has crossed the exposure threshold but shows no signs of heat-related illness.

Active cooling activates when passive cooling is insufficient or a member shows signs of heat-related illness. The standard names forearm immersion and misting fans explicitly among acceptable active methods. In hot, humid conditions, it calls for a minimum of 10 minutes of active cooling, with 20 minutes preferable.

Release from rehab has defined vital sign benchmarks: core temperature below 100.6°F (38.1°C), heart rate below 100 bpm, blood pressure below 160/100, and SpO2 above 94%. A member who does not meet these benchmarks stays in rehab, independent of how the assignment schedule is running.

Laying out a rehab sector that works under pressure

A rehab sector that functions on paper but not on scene usually fails on layout, not on equipment. Four zones, kept physically distinct, make the flow work:

Keeping active cooling staged inside rehab, not as an off-site or secondary resource, is what closes the gap between a rehab sector that has fluids and shade, and one that can actually treat a member who crosses from heat strain into exertional heat stroke.

Why whole-body immersion outperforms the named method

Forearm immersion is the method NFPA 1580 names explicitly, and it is a legitimate active cooling technique. But it covers roughly 12% of total body surface area, versus close to 90% for whole-body immersion. For a member showing real heat illness signs, not just fatigue, that difference determines whether the 10 to 20-minute active cooling window is enough. A rehab sector equipped only for forearm immersion has no answer for a case that needs faster cooling than that method can deliver.

Equipping rehab sectors that move

Structure fires keep rehab close to apparatus, but wildland deployments, mutual aid assignments, and training exercises often put rehab wherever the crew happens to be, with no fixed water source or dedicated support vehicle.
